Job Description: CenterWell Home Health Registered Nurse

This role is for a CenterWell Home Health nurse, joining a caring community focused on putting health first. The position offers an opportunity to make a meaningful impact daily by providing personalized, one-on-one care to help patients regain independence in their homes. The nurse will work closely with a dedicated team of physicians and clinicians to develop and manage care plans that support recovery and help patients return to their desired lives.

Key Responsibilities:

Patient Care & Management: Provide admission, case management, and follow-up skilled nursing visits for home health patients. Administer ongoing care and case management for each patient, with necessary follow-up as directed by the Clinical Manager.
Care Plan Development & Revision: Confer with physicians to develop initial plans of treatment based on physician’s orders and initial patient assessments. Provide hands-on care, management, and evaluation of the care plan, and teach the patient in accordance with physician orders, under the Clinical Manager’s supervision. Revise the plan in consultation with the physician based on ongoing assessments and policy/regulation requirements.
Coordination of Care: Coordinate appropriate care encompassing various healthcare personnel such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers. Report patient care/condition/progress to the patient’s physician and Clinical Manager on a continuous basis.
Patient & Family Engagement: Implement the patient care plan in conjunction with the patient and family to assist them in achieving optimal resolution of needs/problems.
Supervision: Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers. Provide written personal care instructions/care plans that reflect the current plan of care and monitor the appropriate completion of documentation by these workers as part of the supervisory/leadership responsibility.
Discharge & Documentation: Discharge patients after consultation with the physician and Clinical Manager, completing all necessary clinical documentation. Prepare appropriate medical documentation for all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to ensure optimum patient care.

Required Experience/Skills:

Education: Diploma, Associate, or Bachelor Degree in Nursing.
Experience: Minimum of one year nursing experience preferred. Strong experience in Med Surg, ICU, ER, or acute care is required. Home Health experience is a plus.
Licensure & Certifications: Current and unrestricted Registered Nurse licensure; Current CPR certification.
Skills: Strong organizational and communication skills.
Logistics: Valid driver’s license, auto insurance, and reliable transportation.

Pay Range:

Per Visit/Unit: $45.00 – $63.00
Annual Base Pay (for full-time, 40 hours/week): $70,500 – $96,900 per year.
