Job Description: Family Medicine Physician

Job Title: Family Medicine Physician
Location: Lisbon Avenue Health Center in Milwaukee, WI
Employment Type: Full-time
Work Schedule: Does not require working late nights, weekends, or holidays.
Compensation: Dependent upon experience and local market.

The Family Medicine or Internal Medicine Physician is responsible for providing primary health care for individuals in the Primary Care Practice, commensurate with their training and in accordance with the established policies and standards of practice at Progressive Community Health Centers. The role involves delivering care appropriate for an outpatient, primary care community health center serving a diverse multicultural patient population in an urban setting.

Key Responsibilities:

• Provides all physician care typically offered at an ambulatory care facility, on an outpatient basis.
• Responsible for the delivery of care, treatment, and referrals.
• Arranges for referral resources for medical specialties not provided by the corporation for clinic patients.
• Evaluates patients, based on medical training, to determine their medical condition through physical examinations, establishing a diagnosis, and proposing a treatment plan.
• Maintains high standards of medical practice and participates in the training and development of the medical team.
• Assists in the medical education of both clinic patients and staff.
• Facilitates patient transitions within and between health care settings, including referring and transferring patients for specialty care, providing follow-up care, and discharging patients as necessary.
• Fulfills 40 hours of Continuing Education Credits annually.
• Attends professional conventions and seminars, participates in professional societies, and undertakes all reasonable actions to maintain and improve professional skills.
• Counsels patients regarding medical and other problems that might affect their health and well-being.
• Documents each office procedure, prescription, consultation, and patient contact in the patient’s chart and on the clinic’s encounter forms within a 24-hour period.
• Performs clinical procedures for the patient’s benefit that are within the practitioner’s scope and skill and are economically feasible.
• Prepares forms, letters, and examinations on behalf of patients for claims such as disability, workers’ compensation, injuries, and illnesses.
• Provides direction and leadership to the clinic’s medical support staff concerning patient care administration.
• Performs other job-related duties as assigned.

Requirements:

Education: Graduate from an accredited school of Medicine and Residency Program required.
Licensure: Licensed in the State of Wisconsin required.
Board Certification: Board eligibility in Family or Internal Medicine required; Board certification preferred.
Experience: Previous experience in a Family or Internal Medicine setting preferred.

Additional Information:
