Key Responsibilities:

• Personal Care Assistance: Assist clients with daily living activities, including bathing, dressing, grooming, toileting, and feeding, always prioritizing client dignity and comfort.
• Health Monitoring & Reporting: Observe and accurately monitor clients’ health status, vital signs (if applicable and trained), and overall well-being. Promptly report any significant changes, concerns, or unusual observations to the supervising nurse or case manager.
• Companionship & Emotional Support: Provide empathetic companionship, engage clients in conversation, and offer emotional encouragement to foster a positive and supportive environment.
• Environmental Maintenance: Ensure a safe, clean, and organized living environment for clients, which may include light housekeeping, tidying living areas, and assisting with laundry.
• Care Plan Adherence & Communication: Strictly follow individualized care plans developed by healthcare professionals. Maintain open and effective communication with clients, their families, and the healthcare team to ensure coordinated and consistent care.
• Mobility Assistance: Aid clients with safe transfers, ambulation, and exercises as outlined in their care plan, using proper body mechanics and assistive devices when necessary.
