Job Description:
This is a grant-funded Community Health Worker (CHW) Training Program, not an employment opportunity (employment is not guaranteed upon completion). The program aims to equip participants with the essential concepts and skills needed to function as a Community Health Worker, bridging the gap between healthcare, social services, and the community. Upon completion, participants will receive the credit hours necessary to apply for CHW licensure in Oregon.
Key Aspects of the Training Program:
• Focus: Equipping participants with the knowledge and skills to become Community Health Workers.
• Target Audience: Individuals committed to helping patients and their families access quality healthcare and community support. Designed for individuals with minimal or no prior healthcare experience.
• Location: Service Site is in Hillsboro, Oregon. Qualified Candidates will live within 30-45-minute.
• Structure: Combines online classroom training with on-site, hands-on internship/externship training.
• Duration: Approximately 550 hours, with roughly 150 hours of didactic learning and 400 hours of in-person internship.
• Schedule: Monday-Friday, 8 AM-5 PM for the program’s duration.
• Stipend: Total stipend of $7,500, distributed over approximately 16 weeks.
• Topics Covered: History of Community Health Workers, Health Promotion & Disease Prevention, Health Literacy as a Barrier, Telehealth Literacy, Broad Determinants of Health, Health Disparities, Cultural Health Beliefs, Communication Techniques, Managing Referrals, Healthcare Financing, Legal Advocacy, Documentation, Safety Concerns, Screening for Social Risk Factors, EHR Systems & Population Health Management, Technology in Community Care, Social Service Resource Locators.

Training Program Eligibility Requirements:
• Must be 18 years or older.
• Must have no prior professional or volunteer experience in healthcare/community health work (no healthcare licenses/certifications).
• Participants must live within 30-45 minutes from the Service site location of Hillsboro, OR
• Must have a high school diploma or GED.
• Must be a U.S. Citizen or Permanent Resident/Green Card holder (not open to non-citizens or Visa holders).
• Familiarity with Microsoft Office Suite is beneficial.
• Must be able to pass a national criminal background check.
Community Health Worker (CHW) Duties (post-training):
• Establish trusting relationships with patients and their families.
• Coach patients in managing chronic health conditions and self-care.
• Explain program services and impact.
• Assist patients in understanding care plans.
• Potentially visit patients in their homes.
• Document activities and results.
• Advocate for patients and act as a liaison between the patient/family and community service agencies.
• Help patients utilize resources and complete program applications.
• Assist patients in accessing health-related services.
• Facilitate communication between providers.
• Motivate patients to be active participants in their health.
• Effectively work with diverse populations.
• Maintain positive working relationships with various stakeholders.
• Continuously expand knowledge of community resources.
• Other duties as assigned.
Additional Requirements:
• Must be vaccinated with a COVID-19 vaccine (with potential exemptions for medical or religious reasons).
• For on-site internships, participants must meet host site immunization requirements (e.g., influenza, hepatitis B, MMR).
• May need to complete tuberculosis (Tb) testing and drug screening.
