Job Title: Clinical Evaluator (PASRR)

Job Summary:
Acentra Health is seeking Clinical Evaluators (PASRR) to join their team. This role is crucial for ensuring individuals receive appropriate care by conducting face-to-face assessments in Dodge City, Kansas, and surrounding areas. The primary responsibility involves performing PASRR Level II assessments to determine suitable placements and next levels of care for individuals with complex needs. These evaluations focus on individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) across various settings, including nursing homes, hospitals, and community-based environments. The ultimate goal is to improve patient well-being and support better health outcomes.

Role Type and Location:
This is a PRN hybrid role, offering flexibility by combining remote work from a home office with local travel as needed. All travel expenses, including mileage and parking, are covered and reimbursed by Acentra Health. The company also provides necessary technology and equipment for the home office, including a laptop for field use.

Key Responsibilities:

Conduct accurate screenings and evaluations to develop informed level-of-care recommendations based on clinical assessments.
• Perform assessments in diverse environments such as nursing homes, hospitals, and community settings.
• Adhere to state’s best standards in mental health practices, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care.
• Ensure compliance with contractual and regulatory standards during the development of level-of-care recommendations.
• Utilize independent judgment, deep clinical expertise, exceptional communication skills, and problem-solving abilities to manage complex situations and achieve optimal outcomes for members, providers, and clients.
Collaborate effectively with a wide range of healthcare professionals, patients, families, and support networks.
• Monitor personal metrics to ensure compliance with turnaround time requirements.
Leverage technology by using the state’s electronic system and proprietary platforms for efficient management and tracking of assessment activities.
• Stay informed on current literature, regulations, and research related to long-term care, mental illness, medical conditions, and geriatrics.
• Ensure compliance with necessary workflows for quality and efficiency.
Prioritize and execute tasks efficiently with keen attention to detail.

Required Qualifications, Skills, Knowledge, and Experience:

Active, unrestricted licensure in the state of Kansas in one of the following fields:
• Licensed Clinical Social Worker (LCSW)
• Licensed Master Social Worker (LMSW)
• Licensed Marriage and Family Therapist (LMFT)
• Licensed Independent Clinical Social Worker (LICSW)
• Licensed Clinical Psychologist (LCP)
• Registered Nurse (RN)
• Another qualified Licensed Mental Health Professional (LMHP)
Graduation from an accredited college or university with a relevant degree.
Minimum of 2 years of experience assessing and diagnosing individuals with mental or psychiatric disorders, including expertise in complex differential diagnoses involving substance use, medical conditions, cognitive impairments, and dementia.
Minimum 1 year of experience working with individuals with intellectual disabilities or related conditions.
Working knowledge of Preadmission Screening and Resident Review (PASRR) requirements as per Federal Regulation and state PASRR rules.
• Skilled in assessing and analyzing psychological, emotional, behavioral, and physical functioning to develop appropriate level-of-care recommendations.
• Knowledge of psychiatric disorders as outlined by the latest Diagnostic and Statistical Manual in use in Kansas.
Strong communication skills (verbal, written, and electronic) for effective interaction with individuals and groups.
Detail-oriented with the ability to prioritize and execute tasks efficiently within established timelines.
Proficient in computer-based software and Microsoft Office Suite (Word, Excel, Outlook, Teams, etc.).

Preferred Qualifications:

• Familiarity with public mental health and medical programs.
• Prior experience working with the geriatric population.

Compensation:
The compensation for this role ranges from $37.00 to $42.00 per hour, with placement within this range depending on factors such as years of applicable experience and skill level.
