Job Description

The role of a Baylor Registered Nurse, Home Health at CenterWell Home Health involves providing essential skilled nursing care to patients within the comfort of their homes. The primary focus is on facilitating patient recovery and fostering independence. This position requires the development and ongoing management of personalized care plans, carried out in close collaboration with physicians and a multidisciplinary healthcare team. Continuous patient assessment and care coordination are paramount responsibilities. The role also encompasses case management, supervision of home health aides, meticulous documentation, and comprehensive patient education, all while adhering to clinical guidelines.

This position offers the opportunity to make a significant daily impact by delivering personalized, one-on-one care that assists patients in regaining independence within their home environment. Working in close collaboration with a dedicated team of physicians and clinicians, the nurse will develop and manage care plans that support recovery and enable patients to return to their desired quality of life.

As a Home Health Registered Nurse, You Will:

Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer on-going care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
Confer with the physician in developing the initial plan of treatment based on physician’s orders and the initial patient assessment.
• Provide hands-on care, management and evaluation of the care plan, and teaching of the patient in accordance with physician orders, under the Clinical Manager’s supervision.
Revise the plan of treatment in consultation with the physician based on ongoing assessments and as required by policy/regulation.
Coordinate appropriate care, encompassing various healthcare personnel such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
Report patient care/condition/progress to the patient’s physician and Clinical Manager on a continuous basis.
Implement the patient care plan in conjunction with the patient and family to assist them in achieving optimal resolution of needs/problems.
Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing written personal care instructions/care plan that reflect the current plan of care.
Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
• Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimum patient care.

Required Experience/Skills:

To make an impact in this role, candidates should possess:

Education: A Diploma, Associate, or Bachelor Degree in Nursing.
Nursing Experience: A minimum of one year of nursing experience is preferred. Strong experience in med surg, ICU, ER, or other acute care settings is highly valued. Home Health experience is considered a significant plus.
Licensure and Certifications: A current and unrestricted Registered Nurse licensure, along with current CPR certification, is mandatory.
Professional Skills: Strong organizational and communication skills are essential for effective patient care and team collaboration.
Logistics: A valid driver’s license, auto insurance, and reliable transportation are required for home visits.

Pay Range:

Per visit/unit: $45.00 – $63.00.
Per year base pay: $70,500 – $96,900 for a scheduled weekly work of 32 hours. The compensation range provided is an estimated starting base pay for full-time employment and may vary based on geographic location and individual qualifications such as demonstrated job-related skills, knowledge, experience, education, and certifications.
