Job description, detailed

This role is for a Home Care Registered Nurse located in Bronx, NY.

Incentive:
• A $15,000 Sign-On Bonus or Student Loan Assistance is offered.

Role Summary:
The ideal candidate is a compassionate professional with strong clinical insight and decision-making capabilities. This individual will be responsible for mapping out and overseeing a patient’s recovery process from illness, injury, or surgery within their home environment. Patients will rely heavily on the nurse’s judgment and expertise.

Key Responsibilities:
• Provide a full range of advanced clinical home care services.
• Conduct patient assessments.
• Perform medication management.
• Provide patient and family education.
• Assume the ongoing, primary responsibility for coordinating, implementing, and continually evaluating the home care needs of assigned patients.
• Review patient insurance information to ensure comprehensive care planning.
• Coordinate all activities involved in each individual care plan.
• Ensure that pertinent clinical findings are effectively shared with family members and other professional caregivers involved in the patient’s care.

Qualifications:
• Must hold a current NYS RN license and registration.
• A Bachelor’s degree in nursing is preferred.
• A minimum of one year of acute medical-surgical nursing experience is required.
CHHA (Certified Home Health Aide) experience is preferred.
• A valid driver license and car are required for this position.
