Job Description
The Registered Nurse, Home Health at CenterWell Home Health plays a pivotal role in delivering personalized, skilled nursing care and comprehensive case management to patients within their home environments. This position is central to facilitating patient recovery and fostering independence. The nurse works collaboratively with physicians and a multidisciplinary team to develop, implement, and continuously revise individualized care plans. A key aspect of the role also involves supervising home health aides to ensure proper patient care. Core responsibilities include thorough patient assessment, coordination of various healthcare services, meticulous documentation, and crucial education for both patients and their families, all aimed at achieving optimal health outcomes.

You will work closely with a dedicated team of physicians and other clinicians to develop and manage care plans, ultimately supporting patient recovery and helping them return to a fulfilling life.
As a Home Health Registered Nurse, You Will:
• Provide initial and ongoing patient care: Conduct admission, case management, and follow-up skilled nursing visits for home health patients.
• Manage patient care plans: Administer ongoing care and case management for each patient, including necessary follow-up as directed by the Clinical Manager.
• Collaborate on treatment plans: Confer with physicians to develop the initial plan of treatment based on physician’s orders and the initial patient assessment.
• Execute and evaluate care: Provide hands-on care, manage and evaluate the care plan, and educate the patient in accordance with physician orders, under the supervision of the Clinical Manager.
• Revise care plans: Update the plan in consultation with the physician based on ongoing assessments and as required by policy or regulation.
• Coordinate interdisciplinary care: Ensure appropriate care coordination, involving various healthcare personnel such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
• Report patient status: Continuously report patient care, condition, and progress to the patient’s physician and Clinical Manager.
• Empower patients and families: Implement the patient care plan in conjunction with the patient and family to assist them in achieving optimal resolution of their needs and problems.
• Supervise support staff: Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers. This includes providing written personal care instructions/care plans that reflect the current plan of care and monitoring the appropriate completion of documentation by these workers as part of the supervisory and leadership responsibility.
• Manage patient discharge: Discharge patients after consultation with the physician and Clinical Manager, preparing and completing all necessary clinical documentation.
• Maintain comprehensive documentation: Prepare appropriate medical documentation for all patients, which includes details on case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. This also involves preparing visit/shift reports, updating/summarizing patient records, and conferring with other healthcare disciplines to ensure optimum patient care.
Required Experience/Skills:
• Education: Diploma, Associate, or Bachelor Degree in Nursing.
• Nursing Experience: Minimum of one year of nursing experience preferred, with strong experience in Med Surg, ICU, ER, or acute care. Home Health experience is a plus.
• Licensure & Certification: Current and unrestricted Registered Nurse licensure, along with current CPR certification.
• Key Competencies: Strong organizational and communication skills.
• Transportation: Valid driver’s license, auto insurance, and reliable transportation.
Compensation:
The role offers a flexible compensation structure, including:
• Pay per visit/unit: $45.00 – $63.00
• Annual base pay: $70,500 – $96,900 per year (for full-time 40 hours per week employment). The exact pay may vary based on geographic location, demonstrated job-related skills, knowledge, experience, education, and certifications.
Scheduled Weekly Hours:
• 40 hours per week for full-time employment.
