Job Description: Licensed Practical Nurse Home Health

Summary:
The Licensed Practical Nurse (LPN) in home health at CenterWell Home Health plays a crucial role in providing personalized nursing care to patients directly in their homes. This position supports patient recovery and promotes independence under the direct supervision of Registered Nurses and physicians. LPNs deliver skilled nursing interventions, assist in care coordination, meticulously maintain documentation, and ensure strict adherence to professional and regulatory standards. The role demands clinical experience, current state licensure, and strong collaborative skills within a multidisciplinary healthcare team, all aimed at enhancing patient well-being and recovery. As a CenterWell Home Health nurse, you will make a meaningful impact daily by providing one-on-one care that empowers patients to regain independence in the comfort of their own homes. You will collaborate closely with a dedicated team of physicians and clinicians to develop and manage care plans that facilitate recovery and help patients return to their desired quality of life.

Key Responsibilities:
As a Home Health LPN, you will:
Provide Professional Nursing Services: Deliver professional nursing care under the direction of a Registered Nurse (RN), ensuring full compliance with the state’s Nurse Practice Act, all applicable licensure/certification requirements, healthcare standards, governmental laws and regulations, and CenterWell Home Health policies and procedures.
Deliver Skilled Nursing Interventions: Administer skilled nursing interventions for the treatment of patient illnesses, rehabilitative needs, and preventative care. This involves utilizing a holistic approach to provide problem-specific therapeutic interventions, teaching, and training activities in line with the established plan of care.
Apply Clinical Knowledge: Utilize clinical knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
Confer and Communicate with RN Supervisor: Consult with the RN Supervisor regarding any necessary changes to the Plan of Treatment. Accept verbal orders from physicians when permitted by state law, regulations, or the Nurse Practice Act, and promptly communicate these orders to the RN Supervisor.
Implement Nursing Plan of Care: Employ a systematic, individualized, goal/outcome-driven approach when implementing the nursing plan of care.
Maintain Timely Communication: Keep timely contact with patients, physicians, clinical manager(s), and other members of the healthcare team regarding patient/family needs and status changes. Participate in care coordination activities and discharge planning as appropriate.
Maintain Confidentiality: Uphold the highest standards of professional conduct concerning confidential information. Share information only when the recipient’s right to access is clearly established and when sharing such information is unequivocally in the best interests of the patient.
Ensure Resource Availability: Communicate appropriately to ensure adherence to professional standards regarding the provision and availability of supplies, materials, and equipment necessary to safely and effectively implement the plan of care.
Documentation: Prepare, submit, and maintain all required documentation as mandated by the Company and/or facility. Visit/shift notes must be documented on the same day services are rendered.

Required Experience/Skills:
To qualify for this position, candidates must possess:
Education: Graduation from an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Licensure: A current nursing license in the practicing state.
Transportation: A valid driver’s license, auto insurance, and reliable transportation.
Certifications: Current CPR certification.
Experience: Two years of experience as an LPN/LVN in a clinical setting, with a preference for experience in a home health or hospice setting.

Compensation:
Pay Structure: The role offers both a pay per visit/unit rate and a base pay.
Pay per visit/unit: $28.00 – $40.00
Base Pay (per year): $44,600 – $61,400
Scheduled Weekly Hours: 40 hours per week (full-time).
