Job Description: Home Health LPN

As a CenterWell Home Health nurse, you will become part of a caring community focused on putting health first and making a meaningful daily impact. This role involves providing personalized, one-on-one care to help patients regain independence in their homes. You will collaborate closely with a dedicated team of physicians and clinicians to develop and manage care plans, supporting patient recovery and helping them return to their desired lives.

Key Responsibilities of a Home Health LPN:
• Provide professional nursing services under the direct supervision of a Registered Nurse (RN), adhering to state Nurse Practice Acts, licensure/certification requirements, healthcare standards, governmental laws, regulations, and CenterWell Home Health policies.
• Deliver skilled nursing interventions for patients’ illness treatment, rehabilitative needs, and preventative care, using a holistic approach for problem-specific therapeutic interventions, teaching, and training in line with the care plan.
• Apply clinical knowledge and skills in accordance with accepted clinical practice standards to facilitate problem resolution and achieve individualized patient goals and outcomes.
• Confer with the RN Supervisor regarding necessary changes to the Plan of Treatment and accept verbal orders from physicians when permitted by state law/regulations/Nurse Practice Act, communicating these orders to the RN Supervisor.
• Implement the nursing plan of care using a systematic, individualized, goal/outcome-driven approach.
• Maintain timely contact with patients, physicians, clinical manager(s), and other healthcare team members regarding patient/family needs and status changes. Participate in care coordination activities and discharge planning as appropriate.
• Uphold the highest standards of professional conduct regarding confidential information, sharing it only when access rights are established and it is clearly in the patient’s best interest.
• Ensure appropriate communication to maintain professional standards in the provision and availability of supplies, materials, and equipment needed for safe and effective care plan implementation.
• Prepare, submit, and maintain required documentation, ensuring visit/shift notes are documented on the day services are rendered.

Required Experience/Skills:
Education: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Licensure: Current nursing license in the practicing state.
Transportation: Valid driver’s license, auto insurance, and reliable transportation.
Certification: Current CPR certification.
Experience: Two years of experience as an LPN/LVN in a clinical setting, with a preference for experience in a home health or hospice setting.

Compensation:
Pay Structure: Offers both a pay-per-visit/unit rate and a base annual salary.
Pay per visit/unit: $28.00 – $40.00
Base Pay (per year): $44,600 – $61,400 (This range reflects a good faith estimate for full-time employment at the time of posting and may vary based on geographic location, individual skills, knowledge, experience, education, and certifications.)
