Job Description: LPN Home Health Nurse

This role is for an LPN (Licensed Practical Nurse) based in the mid-Cape Hyannis area of Cape Cod, MA, covering a 30-mile radius.

Role Overview:
As a CenterWell Home Health nurse, you will become part of a caring community focused on patient health. You will make a meaningful impact daily by providing personalized, one-on-one care that enables patients to regain independence in the comfort of their homes. Working closely with a dedicated team of physicians and clinicians, you will develop and manage care plans to support recovery and help patients return to their desired lives. A $5,000 sign-on bonus is available for this position.

Key Responsibilities:

Professional Nursing Services: Provide skilled nursing services under the direction of a Registered Nurse (RN), ensuring compliance with the state’s Nurse Practice Act, applicable licensure/certification requirements, healthcare standards, governmental laws, and CenterWell Home Health policies.
Interventions and Approach: Deliver skilled nursing interventions for patients’ illnesses, rehabilitative needs, and preventative care. Utilize a holistic approach for problem-specific therapeutic interventions, teaching, and training activities in alignment with the patient’s plan of care.
Clinical Practice: Apply knowledge and skills according to accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
Collaboration with RN Supervisor: Confer with the RN Supervisor regarding necessary changes in the Plan of Treatment. Accept verbal orders from physicians when permitted by state law/regulations/Nurse Practice Act and communicate these orders to the RN Supervisor.
Care Plan Implementation: Employ a systematic, individualized, goal/outcome-driven approach in implementing the nursing plan of care.
Communication & Coordination: Maintain timely contact with patients, physicians, clinical managers, and other healthcare team members regarding patient/family needs and status changes. Participate in care coordination activities and discharge planning as appropriate.
Confidentiality: Uphold the highest standards of professional conduct concerning confidential information, sharing it only when the recipient’s right to access is clear and it serves the patient’s best interests.
Resource Management: Communicate appropriately to ensure adherence to professional standards regarding the provision and availability of supplies, materials, and equipment needed to safely and effectively implement the plan of care.
Documentation: Prepare, submit, and maintain all required documentation by the Company and/or facility, ensuring visit/shift notes are documented on the day services are rendered.

Required Experience/Skills:

Education: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Licensure: Current nursing license in the practicing state (Massachusetts).
Transportation: Valid driver’s license, auto insurance, and reliable transportation.
Certification: Current CPR certification.
Experience: Two years of experience as an LPN/LVN in a clinical setting, with a preference for experience in a home health or hospice setting.

Compensation:

Pay per visit/unit: $31.00 – $44.00.
Annual Base Pay (Full-time, 40 hours/week): $49,100 – $67,500.
