Job description
As a CenterWell Home Health LPN, you will make a meaningful impact by providing personalized, one-on-one care to help patients regain independence in their homes. You will work closely with a dedicated team of physicians and clinicians to develop and manage care plans that support recovery.
As a Home Health LPN, you will:
• Provide professional nursing services under the direction of an RN, adhering to the state’s Nurse Practice Act, licensure/certification requirements, healthcare standards, governmental laws, regulations, and CenterWell Home Health policies.
• Deliver skilled nursing interventions for patient/client illness, rehabilitative needs, and preventative care. This includes utilizing a holistic approach for problem-specific therapeutic interventions, teaching, and training activities according to the plan of care.
• Apply knowledge and skills in line with accepted clinical practice standards to facilitate problem resolution and achieve individualized patient goals and outcomes.
• Confer with the RN Supervisor regarding necessary changes to the Plan of Treatment. You will accept verbal orders from physicians when permitted by state law/regulations/Nurse Practice Act and communicate these to the RN Supervisor.
• Implement the nursing plan of care using a systematic, individualized, goal/outcome-driven approach.
• Maintain timely contact with patients, physicians, clinical manager(s), and other healthcare team members regarding patient/family needs and status changes. You will participate in care coordination activities and discharge planning as appropriate.
• Uphold the highest standards of professional conduct concerning confidential information, sharing it only when the recipient’s right to access is clear and it is in the patient’s best interest.
• Ensure appropriate communication regarding the adherence to professional standards in the provision and availability of supplies, materials, and equipment needed to safely and effectively implement the plan of care.
• Prepare, submit, and maintain documentation as required by the Company and/or facility, ensuring visit/shift notes are documented on the day services are rendered.
Required Experience/Skills:
• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice environment.
