Job Description

Licensed Practical Nurses (LPNs) in Home Health at CenterWell Home Health provide personalized nursing care to patients in their homes. This role supports patient recovery and independence, always under the supervision of a Registered Nurse (RN). LPNs perform skilled nursing interventions, contribute to the creation and updating of care plans, and collaborate effectively with other healthcare team members to ensure quality patient outcomes. The position requires strict compliance with state nursing regulations and effective communication with patients, physicians, and the entire care team. This role aims to make a meaningful impact by providing one-on-one care that helps patients regain independence in the comfort of their homes, working closely with a dedicated team of physicians and clinicians to develop and manage care plans.

As a Home Health LPN, You Will:

Provide Professional Nursing Services: Deliver nursing services under the direction of an RN, ensuring compliance with the state’s Nurse Practice Act, all applicable licensure/certification requirements, healthcare standards, governmental laws, regulations, and CenterWell Home Health policies and procedures.
Perform Skilled Nursing Interventions: Apply skilled nursing interventions in treating patient illnesses, addressing rehabilitative needs, and providing preventative care. Utilize a holistic approach for problem-specific therapeutic interventions, teaching, and training activities, all in accordance with the established plan of care.
Apply Clinical Knowledge: Utilize knowledge and skills aligned with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
Collaborate on Plan of Treatment: Confer with the RN Supervisor regarding any necessary changes in the Plan of Treatment. Accept verbal orders from physicians when permitted by state law/regulations/Nurse Practice Act, and promptly communicate these orders to the RN Supervisor.
Implement Nursing Plan: Employ a systematic, individualized, goal/outcome-driven approach when implementing the nursing plan of care.
Maintain Communication: Maintain timely contact with patients, physicians, clinical manager(s), and other healthcare team members regarding patient/family needs and changes in status. Participate in care coordination activities and discharge planning as appropriate.
Maintain Confidentiality: Uphold the highest standards of professional conduct concerning confidential information, sharing it only when the recipient’s right to access is clearly established and when sharing is unequivocally in the patient’s best interest.
Ensure Supply Adherence: Communicate appropriately to ensure adherence to professional standards regarding the provision and availability of supplies, materials, and equipment necessary to safely and effectively implement the plan of care.
Manage Documentation: Prepare, submit, and maintain all required documentation as mandated by the Company and/or facility. Visit/shift notes must be documented on the day services are rendered.

Required Experience/Skills:

Education: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Licensure: Current nursing license in the practicing state.
Transportation: Valid driver’s license, auto insurance, and reliable transportation.
Certifications: Current CPR certification.
Experience: Two years of experience as an LPN/LVN in a clinical setting, with a preference for experience in a home health or hospice setting.

Pay and Benefits:

Scheduled Weekly Hours: 40 hours.
Pay Range: The estimated starting base pay for full-time employment (40 hours per week) is $44,600 – $61,400 per year. The pay range may vary based on geographic location, and individual pay will depend on demonstrated job-related skills, knowledge, experience, education, and certifications.
• Alternatively, there is a pay-per-visit/unit option ranging from $28.00 – $40.00.
