Job Description: RN Case Manager, Home Health

Overall Job Summary:
The RN Case Manager in Home Health at CenterWell Home Health provides skilled nursing care and comprehensive case management to patients within the comfort of their homes. This role is crucial in coordinating with physicians and other healthcare professionals to develop, implement, and manage personalized care plans. The ultimate aim is to support patient recovery, foster independence, and help individuals regain their former quality of life through compassionate, one-on-one, home-based care.

Key Responsibilities:

• Conduct admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer and manage the ongoing care for each patient, providing necessary follow-up as directed by the Clinical Manager.
Collaborate with the physician to develop the initial plan of treatment based on physician’s orders and initial patient assessment.
• Provide hands-on patient care, manage and evaluate the care plan, and deliver essential patient education in accordance with physician orders and under the Clinical Manager’s supervision.
Revise the care plan in consultation with the physician based on ongoing assessments and as required by policy/regulation.
Coordinate appropriate care involving various healthcare personnel, including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
• Maintain continuous communication by reporting patient care, condition, and progress to the patient’s physician and Clinical Manager.
Implement the patient care plan in conjunction with the patient and family to help them achieve optimal resolution of their needs and problems.
Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing clear written personal care instructions/care plans that reflect the current plan of care.
Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
Discharge patients after consultation with the physician and Clinical Manager, ensuring all needed clinical documentation is prepared and completed.
Prepare comprehensive medical documentation for all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. This also involves preparing visit/shift reports, updating/summarizing patient records, and conferring with other health care disciplines to ensure optimum patient care.

Required Experience/Skills:

Education: A Diploma, Associate, or Bachelor Degree in Nursing is required.
Nursing Experience: A minimum of one year of nursing experience is preferred.
Clinical Experience: Strong med surg, ICU, ER, or acute experience is highly valued.
Home Health Experience: Prior Home Health experience is a plus.
Licensure: A current and unrestricted Registered Nurse licensure is mandatory.
Certifications: Current CPR certification is required.
Core Competencies: Demonstrated strong organizational and communication skills.
Transportation: A valid driver’s license, auto insurance, and reliable transportation are required for this role.

Pay Range:

Per Visit/Unit Pay: $45.00 – $63.00
Annual Base Pay: $70,500 – $96,900 per year

Actual pay may be higher or lower based on geographic location and individual factors such as demonstrated job-related skills, knowledge, experience, education, and certifications.
