About the Job

This job description is for a CenterWell Home Health LPN (Licensed Practical Nurse). The core purpose of this role is to provide personalized, one-on-one nursing care to patients in the comfort of their homes, helping them regain independence. As an LPN, you will work closely with a dedicated team of physicians and clinicians to develop and manage care plans that support patient recovery.

Key Responsibilities of a Home Health LPN:

Provide professional nursing services under the direction of a Registered Nurse (RN), adhering to state Nurse Practice Acts, licensure requirements, healthcare standards, governmental regulations, and CenterWell Home Health policies.
• Deliver skilled nursing interventions for patient illness, rehabilitative needs, and preventative care, using a holistic approach.
• Implement problem-specific therapeutic interventions, teaching, and training activities in line with the patient’s care plan.
• Apply clinical knowledge and skills to facilitate problem resolution and achieve individualized patient goals and outcomes.
Confer with the RN Supervisor regarding necessary changes to the Plan of Treatment and accept verbal orders from physicians (where permitted by state law/regulations/Nurse Practice Act), communicating these to the RN Supervisor.
• Utilize a systematic, individualized, goal/outcome-driven approach in executing the nursing plan of care.
Maintain timely contact with patients, physicians, clinical managers, and other healthcare team members regarding patient/family needs and status changes.
• Participate in care coordination activities and discharge planning as appropriate.
• Maintain the highest standards of professional conduct regarding confidential information, sharing it only when access is clearly established and beneficial to the patient.
• Ensure adherence to professional standards regarding the availability of supplies, materials, and equipment for safe and effective care implementation.
Prepare, submit, and maintain required documentation, including visit/shift notes documented on the day services are rendered.

Required Experience/Skills for the Role:

Graduation from an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Current nursing license in the practicing state.
Valid driver’s license, auto insurance, and reliable transportation.
Current CPR certification.
Two years of experience as an LPN/LVN in a clinical setting, with a preference for experience in a home health or hospice setting.

The compensation for this role is described as $28.00 – $40.00 per visit/unit or a base annual pay of $44,600 – $61,400 per year.
