Job Description: CenterWell Home Health LPN

This role is part of a caring community focused on putting health first, making a meaningful impact daily as a CenterWell Home Health nurse. The position involves providing personalized, one-on-one care to help patients regain independence in their homes, working closely with physicians and clinicians to develop and manage care plans for recovery.

As a Home Health LPN, You Will:

• Provide professional nursing services under the direction of a Registered Nurse (RN), adhering to the state’s Nurse Practice Act, licensure/certification requirements, healthcare standards, governmental laws, and CenterWell Home Health policies.
• Deliver skilled nursing interventions for patient illness, rehabilitative needs, and preventative care, using a holistic approach for problem-specific therapeutic interventions, teaching, and training in accordance with the care plan.
• Apply clinical practice knowledge and skills to facilitate problem resolution and achieve individualized patient goals.
• Confer with the RN Supervisor regarding necessary changes to the Plan of Treatment and accept verbal orders from physicians when permitted by state law/regulations/Nurse Practice Act, communicating these to the RN Supervisor.
• Implement the nursing plan of care using a systematic, individualized goal/outcome-driven approach.
• Maintain timely contact with patients, physicians, clinical managers, and other healthcare team members regarding patient/family needs and status changes, participating in care coordination and discharge planning as appropriate.
• Maintain the highest standards of professional conduct regarding confidential information, sharing it only when access is clearly established and is in the patient’s best interest.
• Ensure appropriate communication for adherence to professional standards regarding the availability of supplies, materials, and equipment needed for safe and effective care.
• Prepare, submit, and maintain required documentation, ensuring visit/shift notes are documented on the day services are rendered.

Required Experience/Skills:

Education: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Licensure: Current nursing license in the practicing state.
Transportation: Valid driver’s license, auto insurance, and reliable transportation.
Certifications: Current CPR certification.
Experience: Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.

Pay Range:

Per Visit/Unit: $28.00 – $40.00
Annual Base Pay (for full-time employment): $44,600 – $61,400 per year.
