About the Job: Home Care Nurse

This role is for a Registered Nurse looking for a change, specifically in home healthcare. The job description highlights a return to “the HOME” as a way to rekindle love for nursing, emphasizing focused, one-on-one patient care in a comfortable, less distracting setting. It offers a flexible schedule and the opportunity for valuable and meaningful 1:1 time with patients.

Core Responsibilities:
The Home Care Nurse is responsible for providing professional nursing care to patients in their homes, encompassing assessment, development, implementation, and evaluation of home nursing care needs. This is done on an individual and continuous basis, adhering to organizational policies, procedures, and external regulations.

Key Duties Include:
• Ensuring compliance with physician orders regarding care and visit frequency, with documentation reflecting progression towards discharge goals.
• Conducting patient care (assessment, planning, intervention, evaluation) according to policy, procedure, and standard of care, demonstrating holistic care for patients and their significant others.
• Providing safe patient care and demonstrating competency in assigned treatments and procedures.
• Utilizing the interdisciplinary team and accurately documenting all activities.
• Ensuring clinical judgments and critical thinking align with professional standards of care and ethics, leading to safe and effective patient outcomes, with proper documentation of clinical decisions.
• Identifying appropriate payors and maintaining compliance with authorizations for self and other disciplines, utilizing secure messaging for authorizations.
• Completing documentation of assessment, interventions, and planning within organizational timelines, including nursing assessment, medication reconciliation, and collaboration with physicians on the Plan of Care, updating it as patient status changes.
• Implementing specific teaching strategies to meet established goals on the plan of care, ensuring documentation reflects these strategies.
Reporting patient care problems to management and/or physicians promptly with possible solutions, and appropriately utilizing managers as resources.
• Acting as a liaison among physicians, Manager of Clinical Practice, and other healthcare team members for effective coordination of services and appropriate resource referrals.
Communicating pertinent patient information and Plan of Care during transfers of patient care loads.
• Completing routine assignments accurately, consistently, and independently, meeting productivity requirements.
• Acting as a clinical supervisor for para-professional home health aide staff, completing supervisory visits within regulatory timeframes and participating in their evaluation.
• Maintaining awareness of new/updated standards and incorporating them into nursing practice, ensuring documentation reflects current practices’ guidelines.
• Other duties and/or projects as assigned, demonstrating flexibility.
• Adhering to HMH’s Organizational competencies and standards of behavior.
Physical requirements include lifting, pushing, and pulling a minimum of 50 lbs., and standing a minimum of 8 hours a day.

Required Qualifications:
Education: Associate’s Degree in Nursing or Diploma Certificate in Nursing.
Experience: At least six (6) months RN experience OR at least 1 year of LPN experience (preferably in home care or hospice).
Skills: PC Experience/Skills.
Licenses & Certifications: NJ State Professional Registered Nurse License, AHA Basic Health Care Life Support HCP Certification.

Preferred Qualifications:
Education: BSN.
Licenses & Certifications: IV Certification.
