Job Description (Physician Role)
Position Type: This is a Part-time, exempt position, requiring an in-person presence at job locations.
Overview:

Key Responsibilities (Essential Functions & Responsibilities):
• Attending Physician Role: The physician will serve as the attending physician for residents residing at each assigned skilled nursing facility.
• Collaboration & Management: Collaborate closely with Nurse Practitioners (NPs) and Physician Assistants (PAs) to manage the day-to-day resident care at each facility. Typically, each facility will have a full-time PA or NP assigned, depending on patient acuity and volume.
• Patient Care: Provide high-quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs.
• Diagnosis & Treatment: Be responsible for diagnosing and treating patients with both acute illnesses (e.g., bacterial and viral infections) and chronic diseases and conditions (e.g., hypertension, heart disease, or diabetes).
• Documentation: Complete provider documentation efficiently in Theoria’s proprietary electronic health record (EHR) system, ChartEasy™. Familiarity with common facility EHR systems such as PointClickCare, MatrixCare, and American HealthTech is also relevant.
• Quality Initiatives: Actively participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring (RPM), and other medical quality initiatives and programs.
• Supervision: Be able to supervise collaborating nurse practitioners and physician assistants.
• Electronic Tool Utilization: Utilize electronic/online medical tools effectively, including EHR systems and state/county medical systems used for administrative functions like Death Certificates.
Shift Structure:
• Flexibility: Shifts are highly flexible and tailored to the physician’s schedule.
• Minimum Workload: Attending physicians typically work a minimum of 1-2 days per week at each facility they oversee.
• Rounding Times: Rounding times are flexible with no set start and end time, though physicians preferably start in the morning between 7-10 a.m. and finish their day once their caseload has been seen.
• No On-Call/Overnight: There are no nights or on-call duties required. Call responsibilities are covered by Theoria’s dedicated virtual care team, ensuring no overnight call/requirements.
• In-Person: This is an in-person position.
