Job Description: Licensed Practical Nurse (LPN)

As a Licensed Practical Nurse (LPN), you will play a crucial role in providing professional nursing care for assigned patients, working collaboratively under the direct supervision of a Registered Nurse (RN). Your primary responsibility will be to assist in utilizing the nursing process—assessing, planning, implementing, and evaluating patient care—to ensure optimal patient outcomes.

This position requires a commitment to providing age-specific care appropriate to your assigned area, maintaining high standards of practice, and adhering to established policies and procedures. You will be responsible for managing supplies and equipment efficiently, fostering strong collaboration with medical and other healthcare staff, and actively supporting the facility’s mission statement. A key aspect of your role will be achieving customer satisfaction and identifying system problems as opportunities for continuous improvement.

Qualifications

  • Education:
    • Required: Vocational / Technical Degree in Graduate of an accredited school of Practical Nursing.
  • Work Experience:
    • Preferred: 1 – 2 years of Practical Nursing Experience.
  • Licenses and Certifications:
    • Required Certifications/Licensures:
      • Licensed Practical Nurse (LPN) with current Georgia license.
      • Basic Life Support (BLS).
    • Preferred Certifications/Licensures:
      • Advanced Cardiac Life Support (ACLS).
    • Behavioral Health Employees:
      • Required: Crisis Response Training within 2 months of hire.

Essential Functions

LPN – NURSING STANDARDS:

  • Comply with nursing standards under the guidance of the Nursing Staff.
  • Maintain adherence to clinic and hospital policies, procedures, objectives, quality assurance, safety, environmental, and infection control guidelines. Interpret these standards to personnel, patients, and the public as appropriate.
  • Follow established routines and utilize supplies and equipment in a cost-effective manner.

Assessment And Diagnosis:

  • Collect patient data in a systematic and ongoing process.
  • Identify issues and develop diagnoses based on comprehensive assessment data.
  • Prioritize data collection effectively based on the patient’s current condition and needs.
  • Identify expected outcomes and formulate a plan of care based on diagnoses or identified issues.

Care Processes:

  • Treat patients and their families with compassion and implement the established plan of care.
  • Base all care decisions on the documented plan of care.
  • Monitor and accurately document patient responses to interventions.
  • Establish and maintain strong rapport with clients.
  • Adapt communication style, actively listen, and respond effectively to patient needs.
  • Recognize and respond to age-specific responses to treatment.
  • Provide effective interventions as required.
  • Respect patient confidentiality and privacy in all interactions.
  • Support the social, spiritual, and cultural needs of patients and their families.
  • Effectively manage difficult or emotional patient situations with professionalism and empathy.

Outcomes And Evaluations:

  • Develop expected outcomes that provide clear direction for continuity of care.
  • Involve the patient, their family, and other disciplines in determining expected outcomes.
  • Modify expected outcomes based on changes identified during ongoing patient assessment.
  • Include the patient and all involved in the care in the evaluation process.
  • Evaluate the plan of care in relation to patient responses and expected outcomes.
  • Accurately document the results of the evaluation.

Documentation:

  • Document and submit all required information and data in a timely fashion.
  • Clearly and accurately document designated processes, policies, products, service offerings, etc.
  • Ensure that documentation is tailored to expected readers/users.
  • Use correct medical terminology consistently.
  • Conform to required style and format for all documentation.

Additional Duties

  • Adhere strictly to hospital and departmental attendance and punctuality guidelines.
  • Perform all job responsibilities in alignment with the core values, mission, and vision of the organization.
  • Perform other duties as required and complete all job functions as per departmental policies and procedures.
  • Maintain current knowledge in present areas of responsibility through self-education and attendance at ongoing educational programs.
  • Attend staff meetings and complete mandatory in-services, requirements, and competency evaluations on time.
  • Demonstrate competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs.
  • For non-clinical areas, attend training and demonstrate usage of age-specific customer service skills.
  • Wear protective clothing and equipment as appropriate for the role.
