Position Summary

The Primary Care Nurse (PCN) delivers high-quality, comprehensive, and professional nursing care to individuals and families across all life stages within a primary care setting. Working as an integral part of a clinical team, the PCN operates at the top of their licensure to enhance the safety, effectiveness, and efficiency of patient care. This is achieved through the performance of professional nursing responsibilities and the appropriate delegation of technical and clerical functions to medical assistants.

The PCN provides patients with both general and focused health education concerning preventive, chronic, and episodic health care issues. In collaboration with primary care providers, the PCN assumes responsibility for coordinating care, supporting transitions between different levels of care, and counseling, educating, and supporting patients in their journey to improve health status. This role actively applies principles of advanced access, planned care, the chronic care model, and rapid cycle performance improvement using the PDSA approach within CHCI.

Responsibilities

  • Supports primary care for patients under a planned care model, collaborating with Primary Care Providers (PCPs) to identify needs for preventive care, evidence-based chronic disease care, and care coordination.
  • Provides general and specific health care education to patients.
  • Works with patients to identify and monitor self-management goals.
  • Collaborates on patient care with other community-based providers.
  • Conducts follow-up on emergency room visits and hospital discharges to ensure continuity of care.
  • Assists providers with examinations, treatments, and procedures as needed.
  • Ensures the safe, accurate, and clinically appropriate collection, storage, and processing of all biologic specimens.
  • Administers medications, including vaccines and immunizations, orally, subcutaneously, intramuscularly, intradermally, or via nebulizer, strictly adhering to prescribing provider orders and established CHCI standing orders.
  • Adheres to CHC protocols regarding documentation of medication, route, dosage, and information provided to the patient.
  • Monitors patient response to administered medication, detecting, reporting, and responding to any adverse reactions.
  • Provides appropriate patient education related to administered medications.
  • Performs a wide range of nursing activities as needed, such as wound care and catheterization, after initial prescription by a primary care provider.
  • Accurately documents all aspects of patient care in the medical chart on a timely basis.
  • Schedules tests and referrals for specialty care as ordered by the provider.
  • Maintains strict infection control standards in accordance with CHCI protocols, universal precautions, OSHA, and CLIA standards.
  • Acts as a resource person for medical assistants regarding patient preparation, needs, and required procedures.
  • Monitors and ensures follow-up of all abnormal Pap Smear results, STDs, lead screens, and other tests and screenings performed at CHCI.
  • Oversees the maintenance of supplies and equipment, ordering supplies as necessary.
  • Maintains compliance with agency mandatory annual training (Patient Safety, Abuse, Age Based Competencies, EOC, Abuse trainings, etc.).
  • Demonstrates expert use of the electronic health record to ensure timely, safe, and confidential storage and retrieval of patient information.
