This is an unpaid internship position for a Social Work Intern operating under general supervision.
General Responsibilities:
The primary role involves performing professional-level casework by conducting assessments to determine eligibility for the Senior Nutrition program. This includes:
• Receiving referrals via phone, email, or voicemail.
• Documenting referrals in the ServTracker database.
• Initiating contact with potential participants through calls.
• Conducting phone interviews or home visits to complete assessments.
• Performing timely reassessments to ensure continuous service without gaps.
• Making monthly phone contact with meal recipients to confirm meal reception and assess any changes in their health or living conditions.
• Analyzing situations and determining appropriate courses of action, often under stressful conditions.
• Potentially combining investigative and treatment roles, or having separate functions based on departmental structure.
Essential Functions:
• Conducting in-home and telephone assessments for Senior Nutrition Program (SNP) participants as per policy.
• Maintaining current knowledge of governmental and private community resources to refer clients for financial, psychological, emergency, emotional, or other needs.
• Staying informed about social work theory, techniques, and practices, including individual/group behavior, social problems, the aging process, medical/mental illness, family dynamics, coping behavior, crisis intervention, and treatment for various conditions.
• Routinely engaging in phone conversations with high-risk clients identified through screening.
• Answering phone calls from clients and/or their family members regarding meal service inquiries or available resources.
• Documenting direct client interactions, service development, and referrals according to agency requirements, including maintaining case data in computer programs.
• Performing duties under occasional stress, potentially interacting with individuals experiencing stress.
• Adhering strictly to all guidelines, including state and federal laws, program rules, directives, policies, regulations, professional ethics, and quality assurance standards.
• Serving as a member of an inter/multi-disciplinary team.
• Delivering meals to homebound participants as needed.
• Performing other related duties as assigned.
Qualifications:
• Education and Experience:
• Requires current attendance in a Bachelor’s of Social Work (BSW) program with an accredited university.
• Alternatively, a four-year degree in a human services field is acceptable.
• This is an unpaid internship.
• Licenses or Certifications:
• Must possess a Valid Driver’s License with a safe driving history.
